These Terms of Service ("Terms") govern your access to and use of ANVIL Health, Inc.'s website, mobile application, and services (collectively, the "Services"). By using the Services, you agree to these Terms.
Eligibility
You must be at least 18 years old, a resident of the United States, and legally able to enter into a binding contract to use the Services. The Services are not available outside the United States.
Medical disclaimer
ANVIL is a technology platform. The medical care you receive through the Services is provided by independent licensed physicians and is governed by the patient-physician relationship between you and those physicians. ANVIL does not provide medical advice, diagnose conditions, or prescribe medication.

Subscriptions and billing
Our Services are offered on a subscription basis. The intake fee is charged at signup; the monthly subscription begins at the start of your treatment and continues until you cancel. You may cancel at any time through your account or by emailing hello@anvilformen.com.
Refunds are governed by our Refund Policy.
Acceptable use
You agree not to: misrepresent your identity or medical history; resell, share, or transfer medications dispensed to you; use the Services in any unlawful manner; or interfere with the operation of the Services.
Intellectual property
ANVIL owns all right, title, and interest in the Services, including all content, branding, and software. You may not copy, modify, or distribute any portion of the Services without our written consent.
Limitation of liability
To the maximum extent permitted by law, ANVIL shall not be liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the Services.
Governing law
